Patient Partners Coordinator

Reports to: Director of Senior Services Department

Primary Responsibilities: Provide case management and advocacy services to maintain and improve functioning of senior clients and their families. Liaison, train, and coordinate Patient Advocates in their case assignments. Provide outreach, education and networking opportunities with community resources to enhance client/family care related to this program.

Specific Responsibilities:

· Outreach to the community to promote the Patient Partners Program

· Maintain the Policy and Procedure Manuals for the Patient Partners Program

· Training of volunteers enrolled in the Patient Partners Program

· Determine client appropriateness for Patient Partners and full Senior Services Programs

· Oversight of all reporting to client families

· Interface with all departments in the agency relating to the Patient Partners Program

· Develop and maintain ongoing relationships for all volunteers in the program

· Evaluate, train, and match volunteers

· Provide consultations with clients and refer to proper community resources, as needed

· Maintain case records which meet agency, state and COA regulations

· Attend Senior Services and agency wide staff meetings

· Serve as a member on agency committees, including PQI and QAUR, as needed

· Share information regularly with care team and supervisors for client care

· Represent the agency at community programs and outreach events

· Maintain required statistical record and volunteer database

· Community Education

· Facilitate any new agency workshops/programs, as needed

Relationship with Others: Maintain relationship with agency colleagues, as well as program volunteers and clients. Work closely namely with Senior Services Department, and Jewish Federation agencies.


· Degree in Nursing, Masters in Social Work, or similar degree

· Three years’ experience in a health or social service agency/hospital, or institutional setting

· Knowledge of community resources and their development

· Ability to develop training and evaluation mechanisms

· Experience working with elderly and Special Needs populations

· Knowledge of Jewish culture
