Physician - Home Visit


Position Summary:  Physicians in the Home Visit Physicians practice provide primary medical care to patients in their homes and facilities.

Essential Functions:

  1. Provide primary medical care to Home Visit Physician (HVP) patients.
  2. Collaborate with other clinical staff in the HVP practice to insure quality patient care.
  3. Provide appropriate communication and coordination with any of the patient's specialists on an as needed basis.
  4. Maintain open communication with office staff related to scheduling and other patient care needs.
  5. Complete documentation in EMR in timely manner.
  6. Support and participate in performance improvement and quality assurance initiatives in the practice.
  7. Understand and comply with regulations and standards particular to state, federal entities as well as payer rules/regulations.
  8. Review and provide input on clinical policies and
  9. Acts as a liaison between Samaritan Home Visit Physicians and the medical
  10. Conduct community presentations as
  11. Support and collaborate on growth initiatives for the practice with
  12. Receive and resolve patient complaints in collaboration with Director and Practice Manager.
  13. Participate on call activities on a rotating basis.
  14. Enhance professional growth and development through participation in educational programs, literature review, inservices and workshops.
  15. Attend staff meetings on a regular basis.
  16. Accept and perform other related duties and responsibilities as

Requirements

Education or Degree Required:  M.D. or D.O. 

License/Certification Required:  Current license to practice medicine in the state of New Jersey. 

Related Experience and Training Required: At least three years’ experience in the practice of medicine.

Skills and Abilities Required: 

  • Expertise in provision of primary medical care.
  • Excellent communication skills and experience with conflict resolution.
  • Familiar with the principles of quality management and ethical issues in patient care.
  • Ability to use EMR for patient documentation.
