Case Manager
Job Details
The Case Manager ensures individuals are linked to mental health, substance abuse, and medical services as required by project initial assessment. Assists with initial referrals and providing transportation to other programs. Collaborates with other programs individuals are involved with to ensure appropriate care. Participates in on-call rotation for afterhours services. Must have a driver’s license and be able to drive an agency vehicle. Position requires travel to consumer’s residences.
Legacy Treatment Services' Integrated Care Clinic will serve residents of Atlantic County, New Jersey. The project will provide outpatient mental health and substance use counseling, person-centered treatment planning, case management services, peer support services, outpatient primary care screening and monitoring services, services for veterans, and crisis services.
Hours:
This is a 40 hour full time position, Monday – Friday, with some evenings required.

Education/Experience:
Bachelor’s degree in Psychology, Social Work, or related field. Must have one year experience working with psychiatric population. Master’s degree preferred.
