Elder Care Coordinator

We are seeking two dynamic, full-time Elder Care Coordinators who are passionate about serving clients. The ideal candidates will support clients and our firm in various capacities:
Perform care assessments to identify and address potential issues effectively.
Act as a continuous resource, offering guidance, and reassessment of care plans as needed.
Facilitate the hiring of in-home assistance and other necessary services.
Collaborate with medical professionals for comprehensive care coordination.
Assess medical concerns and facilitate referrals to specialized geriatric services while optimizing financial resources.
Assist in planning for potential incapacity or disability.
Aid in the execution of legal documentation.
Offer steadfast support and advocacy during challenging situations.
Organize alternate housing arrangements if required.
Coordinate client transportation to various care facilities.
Provide informative sessions and emotional support.
Guide clients through Medicaid, VA applications, and legal proceedings.
Report to the Director of Care Coordination, working closely with multidisciplinary teams.
Act as a community liaison, promoting the firm's holistic approach to elder law.
Requirements:
PA or NJ LSW, MSW preferred.
Minimum of 3-5 years geriatric case management and or geriatric social work experience.
Comprehensive knowledge of the long-term care system and services/resources including housing in the Delaware, Montgomery, and Chester County areas.
Strong work ethic.
Outstanding interpersonal skills.
Ability to interact effectively with staff, families, and other long-term care professionals.
Demonstrated ability to lead change and effectively manage conflict and crisis situations.
Maintain current professional licensure.
Excellent planning, organizational, budgeting, and interpersonal skills.
Experience and understanding of government and entitlement benefits, Social Security, Medicare, Medicaid, etc.
Valid PA and or NJ driver’s license and proof of insurance required.
Extensive travel within the Southern New Jersey and Delaware, Chester, Bucks, Montgomery, and Philadelphia area required.
