Life Skills Coach
Job Details
Provides youth with in-person therapeutic support services weekly, including personal development skills, social skills, cognitive skills, health and safety, vocational skills, and independent living skills. Facilitates individual and group sessions with youth in the home, office and community settings who have a mental health diagnosis and are intellectually and/or developmentally disabled. Maintains contact with referring agencies and other providers and family/significant others. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. The individual demonstrates knowledge of the principles of growth and development appropriate to the consumer population served. He or she must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group.
- Provide youth with in-person therapeutic support services such as recreation, life skills training, problem solving skills and anger management skills.
- Ability to safely drive clients to and from treatment activities contained in the treatment plan.
- Model professional behavior, demonstrate rapport-building and maintain appropriate boundaries with clients and conform to CTS standards and codes of conduct.
- Respond to client crisis with prescribed procedures for de-escalation, crisis management and behavior management. Demonstrate a trauma-informed response to clients based on Safe Crisis Management guidelines and the Nurtured Heart Approach.
- Implement and document supportive interventions identified in the treatment plan.
- Participates in treatment team meetings as scheduled and maintains contact with youth’s providers.
- Communicate observations both verbally and in the client’s electronic health record using prescribed formats and terminology.
- Provide “on call” support on a rotating basis for after hours emergencies.
- Assist treatment team with the school enrollment process, including gathering documents and maintaining contract with school officials
- Participate in and accept supervision as scheduled with clinical leaders.
- Attend CTS and external trainings in pursuit of professional development and career growth.
- Follow CTS agency policies and procedures regarding work practices and expectations. Ensure client health and medical care needs are met according to Bureau of Licensing regulations.
- Ability to support a youth’s spiritual and cultural background, gender identity and sexual orientation and practices through culturally competent behaviors.

Position is full time and includes the following:

Education/Experience:
Bachelor’s degree in mental health field, social work, or related field preferred. High school graduate/GED with 3-5 years’ experience providing direct care to youth in a behavioral health agency or institutional setting.
