LPN (Licensed Practical Nurse)

JOB SUMMARY:

Assumes responsibility for individual medical case management of agency individuals on a case-by-case basis. Responsible for ensuring the quality of medical care provided meets DDD and Agency standards while working cooperatively with the Medical Case Manager, Director of Residential and the Executive Director to uphold agency and licensing regulations. Ocean County (Toms River, NJ area)

ESSENTIAL JOB FUNCTIONS:

  • Participates in IHP meetings and monitors individual progress of the individual as needed.
  • In preparation for annual IHP, provides a written summary regarding annual medical assessment, current medical condition, medicine regimen and care/service plans and goals for coming year.
  • Monitors the health of the individuals, including the following:
  • Assess the individual’s medication regime, reviews the physician’s plan of care and interprets same to staff.
  • Monitors the results of blood tests and other diagnostic examinations.
  • Assesses the health status of the individuals, etc.
  • Develops a plan of patient care based on the medically prescribed treatment plan, medical assessment, input of individual/family and other associated health providers.
  • Assists with the development and implementation of agency referral system that prioritizes individual needs.
  • Assesses the educational needs of the staff and conducts or arranges for appropriate in-service training programs as trained and guided by the Medical Case Manager.
  • Participates in on going education and training related to the organization and program specific requirements.
  • Instructs and supervises individual care techniques to staff.
  • Attends meetings as needed.
  • Locates health/medical providers to obtain quality medical/health care/treatment for participants.
  • Maintains necessary records and required forms on a timely basis using appropriate documentation.
  • Observes, records and reports signs and symptoms of the recovery process, changes in individual’s physical, emotional and social condition and responses to the therapeutic regiment.
  • Records progress notes that are stored at program sites with copies forwarded to the appropriate supervisors.
  • Reviews professional/medical reports for all individuals.
  • Accompanies individuals on selected medical appointment as deemed necessary.
  • Advocates in the best interest of the individuals at all times.
  • Attends all mandatory Agency training and other training as is identified by the Director of Residential Services
  • Maintains the confidentiality, security and integrity for the management of individual information according to agency and DDD policies
  • Communicates and collaborates with other team members and community agencies in planning, implementing and coordination and evaluating all services necessary to insure comprehensive care.
  • Cooperates fully with licensee and Division staff in any inspection or investigation.
  • Performs other duties as may be assigned by supervisor, Director of Residential Services and/or the Executive Director.

QUALIFICATIONS/EDUCATION:

  • Must be at least 18 years old, have a valid NJ Driver’s License with an acceptable record of safe driving.
  • High School Diploma / GED (verifiable).

PLUS:

  • Valid license as a Licensed Practical Nurse in New Jersey.
  • 2 (two) years experience in the nursing field.
  • Ability to determine the needs and abilities of the individuals and solve problems systematically.
